Title

ANTIMICROBIAL SUSCEPTIBILITY PATTERN OF GRAM POSITIVE ISOLATE IN AN ADULT ICU.

Abstract

ABSTRACT- Infections play vital role in determining the outcome as well as cost and duration of the hospital stay for patients admitted in Intensive Care Unit (ICU) setup. One of the major problems Worldwide is the rise in Antibiotic-Resistant Strains of bacteria, mainly in hospitals. AIMS AND OBJECTIVES: 1) To isolate and identify bacterial pathogens in adult ICU. 2) To study the antimicrobial susceptibility pattern of gram positive isolates. 3) To detect methicillin and vancomycin resistance in Staphylococcus isolates. 4) To detect vancomycin resistance in Enterococci spp. MATERIAL AND METHODS: The study was carried out in Department of Microbiology at a Medical College Hospital, from December 2015 to May 2017. The data of patients regarding age, sex, registration number, date of specimen collection, brief clinical history, associated co-morbid condition were recorded. Depending on sites of infections various samples were collected and processed as per the standard guidelines. RESULTS- In the present study, infection rate was 35.86% and bacteriological culture positivity was observed in 31.21% of the total samples processed. ICU infection was more common in age group of 51-60 years. Gram negative bacilli were found to be commonest cause of ICU infection. Among Gram-positive organisms, S. aureus (18.46%) was commonest, followed by Enterococci spp and CONS. A total of 37.5% S. aureus isolates were MRSA. None of the Gram-positive isolate was found to be VRSA or VRE. CONCLUSION- A total of 37.5% S. aureus isolates were MRSA. None of the Gram-positive isolate was found to be VRSA or VRE, thus judicial use of vancomycin can limit the development of resistant to vancomycin which is an alarming situation in other places. Antibiotic resistant bacteria are becoming an increasingly difficult problem in ICUs.
