Tooth trauma is a common phenomenon having severe immediate and long term consequences. Most dental injuries in Pediatric age group comprises of avulsion and intrusion. Management and prognosis of this avulsed tooth depends upon extraoral dry span. In this case girl child aged 10 years reported after 24 hours after trauma with avulsed maxillary right central incisor (11) and intruded maxillary left central incisor (21). Radiographic examination ruled out the dento-alveolar fractures. The case was managed by extraoral retrograde MTA apexification and orthograde obturation with guttapercha followed by reimplantation of 11 and extrusion of 21. The patient was given flexible splinting for ten days for 11 and 21 was released from splint after 6 weeks. Avulsed tooth had extraoral dry time more than 24 hrs and hence the anticipated prognosis was low for 11 and 21. On follow up after six months clinical and radiographic examinations revealed rare excellent results like establishment of PDL space with 11. The clinical case and managements are discussed .
