Title

Holistic Management Of Diplopia Caused By Lateral Rectus Palsy Through Ayurveda-Case Report

Abstract

Introduction: Diplopia, a condition colloquially referred to as double vision, entails the perplexing perception of two images of a solitary object, which may manifest in a horizontal, vertical, or diagonal arrangement. This medical anomaly is primarily precipitated by the malfunctioning of the extra ocular muscles, rendering both eyes functional yet unable to precisely converge on the intended object. Patients with Lateral Rectus (LR) palsy which is supplied by Abducens Nerve suffer from diplopia. Through diplopia is not described as independent diseases entity in Ayurveda, Acharya Susrutha said, when Doshas enter the third layer of the eye, there may occur a condition known as Timira and if this Doshas settle in the Madhya Bhaga of the eye, it can lead to Dwandwa Drusti or double vision. Early management through Ayurveda can make a great impact. Materials & Methods: This report presents a case of isolated left LR palsy caused by cold wind exposure while travelling, is treated with Ayurvedic treatment approach. A 40 year old female with known case of Diabetic Mellitus for 10 years consulted Shalakya Tantra OPD with a history of sudden onset of difficulty in turning left eye (OS: oculus sinister) away from the nose and diplopia after exposure to cold wind while travelling. Lateral rectus palsy or Dwitiya Patalagata Timira was diagnosed based on history and clinical evaluation. The clinical features of diplopia simulated to Vata Dosha and the treatment modalities adopted were based on Doshas and Samprapti. The patient was treated with oral Ayurvedic drugs and topical medication. Eye exercises after Abyanga and Sweda were also advised. Result: Patient showed satisfactory improvement by the end of management inferring the potential of Ayurveda approaches in management of such conditions. Discussion: The findings highlight the vital importance of Vata in regulating movement and emphasizing its significance in treating Vata related disorders.
