Title

A Comparative study on the efficacy of Hijama bila Shurt (Dry cupping) with Habbe Gule Aakh in the management of Cervical Spondylosis - A Randomised Clinical trial

Abstract

BACKGROUND AND OBJECTIVES - Cervical spondylosis is a disease that encompasses a wide range of progressive degenerative changes that affect all the components of the cervical spine: intervertebral discs, facet joints, ligamentum flavum and laminae. In Unani literature, Waja ‘al-Unq has been described as a localized form of Waja ‘al-Mafasil which is characterized by neck pain and stiffness with or without restricted neck movements. Although Western medicine treats different arthropathies, it lacks effective and safe drugs. Therefore, in the Unani system of medicine, different Regimental modalities like Dalk (massage), fasd (venesection), Inkibab (medicated steam), Naṭūl (irrigation), Ḥijama (cupping) and Riyḍat (Exercise) etc. are found to be very effective. Due to the effectiveness of Unani treatment, it seems rational to evaluate the efficacy of Hijama Bila Shurṭ (dry cupping), and the Unani formulation Habbe Gule Aakh in the management of Cervical Spondylosis. MATERIAL AND METHOD – The study was conducted as a comparative, open label, randomized clinical trial on 60 patients, with 30 in two groups. Both groups received treatment for 28 days. The control group (Group A) received 2 tablets of Habbe Gule Aakh (125 mg) orally twice daily after a meal for four weeks along with active exercises of the neck and the Test group (Group B) received 2 tablets of Habbe Gule Aakh (125 mg) orally after a meal for four weeks with the application of Hijama Bila Shurṭ (dry cupping) for 10-15 minutes along with active exercises of the neck. The pre and post-treatment effects were assessed based on subjective and objective parameters. RESULT AND CONCLUSION - The subjective parameters like pain in the neck and shoulder, tenderness and stiffness, restricted movement of the neck, local swelling and vertigo or dizziness were showed highly significant results when analyzed within the groups with a p-value of p < 0.0001, but there is no significant result between the groups. The objective parameters range of motion such as flexion, extension, neck side flexion and rotation and NDI (Neck Disability Index) show significant results when compared within and between the groups with a p-value of p < 0.05 except for VAS. Therefore, it can be concluded that the regimen Ḥijama Bila Shurṭ along with Habbe Gule Aakh was effective in the management of cervical spondylosis. However, Ḥijama bila Shurṭ was found to be comparatively more effective than Habbe Gule Aakh using the student ‘t' test. Moreover, both groups were found safe without any adverse effects.
