Title

“A RANDOMIZED CONTROLLED CLINICAL STUDY TO EVALUATE THE EFFECT OF AGNIMUKHA CHURNA IN UDAVARTHINI YONIYAPAD (PRIMARY DYSMENORRHOEA)”

Abstract

ABSTRACT Menstruation is a natural event as a part of normal process of reproductive life in females. Primary dysmenorrhea refers to recurrent, spasmodic pain in the lower abdomen beginning with the onset of menstruation and lasting from 12 to 24 hours in the absence of pelvic pathology. In India young adults of age 17-24 years with a higher prevalence range of 84.2%. In Ayurveda classics, out of 20 Yonivyapad, Udavartini is one which is characterised by painful menstruation and can be correlated with primary dysmenorrhea. According to Ayurveda vedana is pratyatmalakshana of Vatavikruti. Normal function of apanavata is the expulsion of artava without any discomfort but vikruti of apanavata causes vimargagamana which in turn causes expulsion of Rajah with discomfort. By analysis one can state, this is a result of vikruta apanavata hence all measure capable of normalising vata are indicated. There are many researches proven satisfactory results in the management of Udavartini yonivyapad but selected trial drug is remain untouched, considering the properties of trial drug like vatanulomana, shoolahara .Agnimukha churna has been selected to evaluate its efficacy on Udavartini yonivyapad as mentioned in Yogaratnakara and Sahasrayoga. AIM OF THE STUDY : To evaluate the efficacy of Aginimukha churna in the management of Udavartini Yonivyapad. OBJECTIVES OF THE STUDY: To compare effect of Agnimukha churna with Rajahpravartini vati in the management of Udavartini Yonivyapad. METHODOLOGY: It is a randomized controlled clinical study with a pretest & post-test design. 40 patients fulfilling the diagnostic and inclusion criteria were selected for the comparative study. The selected patients were randomly divided into 2 groups, 20 patients each. PROCEDURE: The selected 20 patients in Group A (control group) were administered Rajahpravarthini Vati 250mg 2tabs orally daily before food twice a day. The selected 20 patients in Group B (trail group) were administered Agnimukha Churna 6gms orally daily before food twice a day. The duration of treatment was from 7 day before menstrual cycle till 3rd day of menstrual cycle for 3 consecutive menstrual cycles. The assessment was done after each cycle on 4th day of cycle for 3 consecutive menstrual cycle & follow-up on 4th day of 3rd menstrual cycle. RESULTS AND INTERPRETATION: The analysis was done statistically using Mann Whitney, Wilcoxon Signed Rank, Repeated measure Anova test to calculate the values within the groups. Pain was assessed on the basis of WaLLID scale, then analysed statistically. The test of significance showed that the efficacy of Rajahpravarthini Vati and Agnimukha Churna in Udavartini is statistically significant within the groups. The test of significance showed that the efficacy of Rajahpravarthini Vati and Agnimukha Churna in Udavartini is no statistically significant difference in between the groups. Therefore, both Rajahpravarthini Vati and Agnimukha Churna are equally effective in Udavartini yonivyapad.
