Title

AYURVEDIC MANAGEMENT OF HYPERSPLENISM (PLIHA UDARA) - A CASE REPORT

Abstract

BACKGROUND: Srotas which means ‘through that, which flows’. Srotas are innumerable which supplies nutrition to the tissue. According to classics among the 13 important srotas the raktavaha srotas are channels which carries rakta dhatu where the mulasthana are yakrut and pilha. The dusti hetu of raktavaha srotas leads to the vriddhi of Pliha. The pliha vriddhi can be correlated with hypersplenism where the excessive splenic function leading to accelerated destruction of circulating cellular element results in anaemia and thrombocytopenia. MATERIALS AND METHODS: A 58 years old male presented with complaints of weight loss, weakness, anorexia, vomiting, hoarseness of voice, black colour faeces, impaired digestion for past 5 months. As per haematological parameters taken on 09/01/2024 Haemoglobin was 5.8 gm/dL, RBC was 2.4 million/Cmm. Haematology blood smear study shows Normocytic normochromic anaemia – Thrombocytopenia. Histopathology bone marrow biopsy details as chronic lympho proliferative disorder – Hypersplenism and was advised for splenectomy. The presenting condition is diagnosis as plihodara and upadrava of pandu where nitya virechana and samana yogas are followed as line of treatment. RESULT: The significant improvement observed during the treatment period. The improvement has been assessed by haematological parameter in a regular interval. DISCUSSION: Spleen is the largest lymphoid organ in the body. Lympho proliferative disorders develop due to immune system dysfunction which leads to spleen enlargement, anaemia, loss of appetite, unintentional weight loss. Hypersplenism is a triad of splenomegaly, pancytopenia, thrombocytopenia. In classics the Raktavaha sroto dusti leads to plihavriddhi where aggravation of rakta and kapha are noted due to intake of vidahi and abhisyandi ahara. Nitya virechana and srotoshodhana dravyas are given regularly to eliminate the excessive dosha and clears the srotas. Nitya virechana works on gross level by cleansing the channels and in cellular level where the purification is produced in the cell membrane and molecules results in raktaprasadana by eliminating dusta kapha and pitta.
