In vivo formation of platelet aggregates was studied in 200 patients with congestive heart failure and 10 normal volunteers. Patients with heart failure had a significantly (p less than 0.01) higher number of circulating platelet aggregates compared to the normal volunteers. During sodium nitroprusside infusion, circulating platelet aggregates returned to normal, and in vitro responsiveness to epinephrine and adenosine diphosphate was also significantly suppressed (p less than 0.01). This was accompanied by a 30% reduction in systemic vascular resistance and a 28% rise in cardiac output. Sodium nitroprusside has direct, dose-dependent and platelet inhibition actions, confirmed in other in vitro tests. This study points to an increased vascular resistance in some heart failure patients being at least in part.
