As human space exploration progresses toward long-duration missions to the Moon, Mars, and beyond, ensuring astronaut health is of paramount importance. Space presents unique physiological, environmental, and logistical challenges that necessitate a reevaluation of how medicines are formulated, stored, delivered, and regulated. This review explores the emerging fields of Astropharmacy and Space Pharmacology, discussing the impact of microgravity and cosmic radiation on drug efficacy, delivery systems, pharmaceutical manufacturing, and astronaut health. It also highlights current research, ongoing challenges, ethical and legal considerations, and the future prospects of space-based medicine
