Medication errors are one of the leading sources of preventable injury in healthcare systems globally. They can occur at any point during the medication-use process and result in adverse drug events, hospitalisation, permanent impairment, or even death. As medicine experts, chemists play an important role in reducing these errors by actively participating in patient care, such as prescription verification, dose calculations, drug interaction checks, patient counselling, and post-discharge follow-up. This study examines the various forms, causes, and effects of drug errors, highlighting chemists' complex involvement in identifying and reducing these risks. It also includes clinical research that supports pharmacist-led treatments, as well as real-world case studies and practical solutions for improving medication safety. A more effective integration of chemists into multidisciplinary healthcare teams is critical to achieve the goal of safer pharmaceutical practices. a lack of communication among healthcare providers, and poor pharmacological expertise are all factors that contribute to Medication errors.
