Title

UPPER GI ENDOSCOPY IN CHOLELITHIASIS: A NECESSITY?

Abstract

UGI Endoscopy in Cholelithiasis : A necessity ? Dr. Omi Bhandari , Dr. Pratik Gond Maj. Gen. Dr. S K Jha VSM (Retd)(professor and head ) RURAL MEDICAL COLLEGE, PRAVARA INSTITUTE OF MEDICAL SCIENCES, LONI omibhandari15@gmail.com MOB: 8600006149 ARSICON REG. NO. 085 Background: The diagnosis of asymptomatic gallbladder stones is frequently made by chance. Cholecystectomy is seen to be the preferred course of therapy for gallstone disease, which is asymptomatic in the majority of patients. Nevertheless, in order to comprehend the residual symptoms following surgery, less attention has been paid to patient selection and the signs of this illness. According to studies, dyspeptic symptoms are the least likely to be resolved by cholecystectomy, with about 25% of patients not experiencing any alleviation from their symptoms after the procedure. A collection of symptoms known as post-cholecystectomy syndrome (PCS) remain following a cholecystectomy. It is classified as late if it appears months or years after surgery and early if it happens within the post-operative phase. The study's goals were to assess the frequency of dyspeptic symptoms in patients with gallstone disease and to highlight the significance of endoscopic examination as a standard pre-operative investigative technique in patients with dyspepsia and gallstones. Methods: A study of 140 patients who had gallstones confirmed by USG and presented with dyspepsia at the OPD or were hospitalised to the surgical department of the RMC Hospital was conducted. An upper gastrointestinal endoscopy was used to look for any large lesions. Results: Gastritis was the most frequent endoscopic finding. The mean age at presentation was 47 years. The female to male ratio in our study was 1:0.8. Conclusions: In 88.6% of patients, significant endoscopic findings were noted. For all gallstone disease patients presenting with dyspepsia, routine upper gastrointestinal endoscopy prior to surgery is advised in order to prevent unnecessary cholecystectomy.
