Title

Psoriasis (Da-us Sadaf) and Ocular Involvement: A Systemic Disease Beyond Dermatological Boundaries

Abstract

Background: Psoriasis (Da-us-Sadaf) is a chronic, immune-mediated inflammatory disorder that primarily affects the skin, yet growing evidence highlights its systemic nature with multisystem involvement, including ocular complications. Ocular manifestations are frequently underrecognized despite their potential to cause significant morbidity. Objective: This paper aims to provide a comprehensive overview of the pathophysiology, clinical spectrum, and implications of ocular involvement in psoriasis, emphasizing the importance of timely recognition and integrated management. Methods: A narrative review of published literature was conducted to summarize current understanding of immunological mechanisms linking cutaneous and ocular disease, clinical features of psoriasis-related eye involvement, and the role of systemic and topical therapies in disease modulation. Findings: Ocular complications associated with psoriasis range from blepharitis, conjunctivitis, and keratitis to uveitis and dry eye disease, often correlating with disease severity and systemic inflammation. Shared immune pathways, particularly dysregulation of Th1/Th17 cells and pro-inflammatory cytokines such as TNF-α and IL-17, underpin the parallel involvement of skin and ocular tissues. Systemic treatments, while effective for cutaneous symptoms, may have variable effects on ocular outcomes, necessitating careful therapeutic consideration. Conclusion: Psoriasis should be recognized as a systemic disease with significant ocular implications. Early ophthalmologic assessment is essential for preventing irreversible vision loss. A multidisciplinary approach involving dermatologists, ophthalmologists, and rheumatologists is crucial for optimal patient outcomes.
