Title

Antidiabetic Herbs and Emerging Role of Polyherbal Antidiabetic Formulation –A Review

Abstract

Diabetes mellitus (DM) is a chronic metabolic disorder characterized by persistent hyperglycemia and associated complications affecting multiple organ systems. Despite advances in pharmacotherapy, limitations such as adverse effects, cost, and residual cardiovascular risk drive continued exploration of complementary therapies. Medicinal plants have been traditionally used in Ayurveda, Unani, and Siddha systems for the management of “Madhumeha” or diabetes. Single herbs like Curcuma longa, Cinnamomum spp., Gymnema sylvestre, Syzygium cumini, and Aegle marmelos demonstrate glycemic benefits through diverse mechanisms such as insulin sensitization, β-cell protection, and inhibition of carbohydrate absorption. Polyherbal formulations—rational combinations of standardized botanicals offer pharmacodynamic synergy, pharmacokinetic enhancement, and dose-sparing potential. Randomized clinical trials show promising adjunctive benefits and promoting the research in such polyherbal formulations for use in diabetes. This review summarizes the role of herbs in diabetes, mechanisms of action, clinical evidence for major antidiabetic plants, and the rationale and safety considerations for polyherbal formulations. It highlights standardization, regulatory, and pharmacovigilance issues, and identifies future directions for integrating polyherbal therapies into evidence-based diabetes management.
