Agni is the fundamental regulatory principle governing digestion, metabolism, transformation, and vitality in Ayurveda. Any deviation in Agni invariably leads to the genesis of disease through the formation of Ama. Gulma is a unique abdominal disorder characterized by a localized mass associated with pain, distension, and disturbed gastrointestinal functions. Although Vata is considered the primary Doṣha responsible for Gulma, its pathological action is invariably initiated and sustained by Agnidushti. This conceptual review critically analyzes the role of Agni in the etiopathogenesis, progression, clinical expression, and management of Gulma based on classical Ayurvedic principles. The review elucidates how Agnimandya initiates Ama formation, which leads to Vatavarana and subsequent Gulma manifestation. The interrelationship between Jatharagni, Dhatvagni, and Dosha dynamics in Gulma is also elaborated.
