Prameha is one of the most important chronic metabolic disorders described in Ayurveda, manifested primarily through abnormalities in urine and systemic derangements in kapha, meda, kleda, and ojas. The Bruhatrayi—Charaka Saṃhitā, Suśruta Saṃhitā, and Aṣṭāṅga Hṛdayam—provides extensive descriptions of Prameha, including its etiology, classification, pathogenesis, clinical manifestations, complications, and therapeutic principles. This review attempts to comprehensively analyze Prameha Vyadhi based on Bruhatrayi references and interpret its relevance in the context of modern metabolic disorders such as diabetes mellitus, dyslipidemia, obesity, and metabolic syndrome. The review also consolidates dietary and lifestyle recommendations, shodhana–shamana therapies, and prognostic markers discussed in classical texts.
