“A COMPARATIVE EVALUATION ON EFFICACY OF AGNI KARMA, MARMA STIMULATION BY HAND & BY TENS (TRANSCUTANEOUS ELECTRICAL NERVE STIMULATION) IN THE MANAGEMENT OF MANYASTAMBHA W.S.R. TO CERVICAL SPONDYLOSIS

Abstract

Cervical spondylosis (Manyastambha) hampers the quality of life & also influences the socio economic condition of person. Along with ageing, the associated risk factors in young patients are sedentary lifestyle, poor posture & occupational factors such as Computer keyboarding, cell phone texting etc. played a large role in increased prevalence of neckpain in past 20 years. In general population, the point prevalence of neck pain ranges from0.4% to 41.5%. The one year incidence ranges from 4.8% to 79.5% & lifetime prevalence may be as high as 86.8%. Acharya Sushruta has also described Agnikarma as an effective procedure .According to Ayurveda,Agnikarma acts against Vata and Kapha Dosha by its Ushna(hot),Tikshna(penetrating),Sukshma (minute),Laghu(small),Vyavayi(quickacting),andVikasi(quicklyspreading)Guna and it breaks Srotoavarodha, produced due to Vata and Kapha, results in relieving pain and inflammation at that site. The heat therapy,which is given at the local or affected area,increases the local blood circulation by vasodilatation, local metabolic process speedup, wasteproducts get excreted, reduce edema, accelerate repair, resulting in decreased intensity of pain. While Marma Chikitsa works on the neuro-endocrine system. The receptors for pain(Nociceptors) are free nerveending which are present in all parts of body except mind.Intense thermal,mechanical,or chemical stimuli can activate the receptors. Actually, due to pressure or stimulation over these points, certain chemical substance and neurotransmitters are release like endorphins and enkephalin which send nerve impulse to brain causing the desired effect but TENS (Transcutaneous Electrical Nerve Stimulation) as a treatment technique is non invasive and has few side effects when compared with drug therapy. TENS (Transcutaneous Electrical Nerve Stimulation) is a method of electrical stimulation which primarily aims to provide a degree of symptomatic pain relief by exciting sensory nerves and thereby stimulating the pain gate mechanism. It causes Mild rhythmic muscle movements that will cause an increase in blood and lymph circulation which leads to reduced interstitial edema and accumulated tissue metabolites, thereby improving the physiological state of muscle which in turn leads to significant reduction in muscle spasm, stiffness and mainly pain.Therefore there is need to explore different alternative methods and also to know their comparative efficacy in the management of cervical spondylosis.
