Agnikarma (therapeutic cautery) and Marma Chikitsa (vital-point therapy) are two para-surgical/therapeutic modalities in classical Ayurveda. While Agnikarma uses controlled thermal application to treat pain and structural disorders, Marma refers to anatomically and energetically important sites where several tissues meet (mamsa, sira, snayu, asthi, sandhi). This review explores how marma knowledge guides the selection, localization, technique, depth, and safety of Agnikarma; summarizes proposed mechanisms (neurovascular, myofascial, nociceptive modulation), clinical indications (snayugata/vata disorders, musculoskeletal pain, frozen shoulder, epicondylitis), contraindications and precautions, and the contemporary evidence base. Integration of marma anatomy into Agnikarma practice improves precision, efficacy, and safety and provides a conceptual bridge between classical texts and modern pain-management approaches.
