Title

A Prospective Study of Functional Outcome for Posterolateral Corner Injury Operated with the Modified Arciero Technique

Abstract

Background The posterolateral corner (PLC) of the knee is a critical stabilizing structure that resists varus stress, external tibial rotation, and posterior tibial translation. Missed or inadequately treated PLC injuries can lead to persistent instability and failure of cruciate ligament reconstructions. Materials and Methods This prospective study was conducted at Narayana Medical College and Hospital, Nellore, from January 2024 to December 2025. Eight patients with confirmed posterolateral corner injuries underwent surgical reconstruction using the Modified Arciero technique. Functional outcomes were assessed using the Visual Analog Scale (VAS), International Knee Documentation Committee (IKDC) score, and Lysholm knee score preoperatively and at 6 months postoperatively. Results At 6 months follow-up, there was a statistically significant improvement in all functional outcome measures. Mean VAS score improved from 8.38 to 1.38, Lysholm score from 35.5 to 88.75, and IKDC score from 41.88 to 89.38 (p < 0.001). All patients achieved good knee stability and functional recovery with no major complications. Conclusion Posterolateral corner reconstruction using the Modified Arciero technique provides excellent short-term functional outcomes with significant pain relief and restoration of knee stability. Early diagnosis and anatomical reconstruction of PLC injuries are essential for optimal clinical results.
