Biomedical waste management (BMWM) is an essential component of hospital administration and public health practice. Improper segregation, handling, transportation, storage, treatment, and disposal of biomedical waste can increase the risk of infection transmission, occupational hazards, and environmental contamination. This study evaluated hospital waste management practices and compliance with the Biomedical Waste Management Rules (2016) in a selected rural tertiary teaching hospital. A descriptive cross-sectional quantitative study was conducted among healthcare workers and waste-generating departments. A total of 385 participants were included. Data were collected using a structured questionnaire and observation checklist assessing biomedical waste management practices, compliance indicators, and training requirements. Data were analyzed using Microsoft Excel and R software; descriptive statistics and inferential tests including ANOVA and Welch’s t-test were applied. Among 385 participants, 53.0% were female and 47.0% were male. The mean age was reported as 29.9 ± 7.73 years and mean work experience was 3.93 years. Only 35.3% had attended biomedical waste management training. Department-wise compliance ranged from 63.9% to 93.6%, while the overall mean compliance score was 69.4%. A statistically significant difference in compliance was observed across departments (F = 5.99, p = 0.014). In addition, 94.8% of valid respondents reported the need for additional training, and training attendance was significantly associated with compliance (t = 2.75, p = 0.006). The study indicates moderate compliance with Biomedical Waste Management Rules in the selected hospital, with variation between departments. Regular training, monitoring, documentation, departmental supervision, and strengthened institutional support may improve biomedical waste management practices and safer hospital environments.
