Title

Physiotherapy management for general health status of a carcinoma patient - A case study

Abstract

Carcinoma is defined as the abnormal growth of cells in body organs. It may be categorized according to the type of cell involved, duration of the presence of the abnormal cells in the body, the extent of the spread of the cells and the part affected .There are different signs and symptoms for different type of cancers .Along with this the risk factors are also different depending upon the type of cancer. The main important thing for consideration is that most of the cancers are not diagnosed in acute stages. They are being diagnosed in chronic stage. Along with this the diagnosis and treatment is also very expensive as compared to other diseases. Patient has to undergo with a number of diagnostic test to rule out the presence of the carcinoma. Starting with lab test, endoscopy (depending upon the part involved), MRI scan and BIOPSY. Medical management is also painful and expensive starting with surgery, chemotherapy and radiotherapy (depending upon case to case). Case Description: - A 58 years old male was diagnosed with a chronic throat cancer stage 4 .After diagnosis patient went through medical management followed by chemotherapy and radiotherapy. After post-medical treatment patient got generalized body weakness along with decrease body weight, strength and endurance .But patient tries to manage all the above mentioned complaints with medical management and bed rest. After 2 years of post-treatment patient came to clinic with pain in legs, difficulty in standing and weight bearing activities. So, after the complete assessment followed by history taking and examination. The treatment plan was decided and given to the patient. After the 8 weeks of treatment again through assessment was taken to rule out the outcome measures and significance of the treatment. Intervention: - Total 8 weeks of intervention program was set for the patient. In week 1 patient was advised breathing exercises with active range of motion exercises for cervical, hip, knee and ankle joint. In week 2 all the intervention procedure of week 1 was repeated with increase in repetition of exercises. Isometric exercises of hamstrings, knee and glutei were taught to the patient. Resistive exercises were also started in week 2 to increase the strength of the lower limb along with manual resistive exercises. In week 3 and 4 core muscle activation exercises along with this theraband exercises for lower limb strengthening and stretching was advised to the patient. In week 5 and 6 frenkel exercises in supine, standing and sitting were selected for the treatment process. In week 7 and 8 equilibrium coordination exercises were performed to teach balance and coordination to the patient. Outcome measures: - After the intervention through assessment was again taken to rule out the significant changes .The active and passive range of motion of hip, knee and ankle increased. The Manual muscle strength of lower limb also increased as compare to before .There was increase in sensory grading also in superficial, deep and cortical sensations. The equilibrium coordination test was examined after intervention which also shows a significant result.
